| 說明： |
| 一、 | 依據本府衛生局114年7月3日桃衛疾字第1140059056號函辦理。 |
| 二、 | 為降低本市帶狀疱疹發生風險，倡導「主動守護健康，桃園與您同行」核心理念，提升市民預防醫學與醫療自主意識，從日常生活中落實自我健康管理，主動參與疫苗接種、了解自身健康風險，並提前思考醫療照護選擇，本府衛生局自本年7月1日起辦理旨揭計畫，說明如下： |
| (一) | 實施對象：需設籍本市，並同時符合下列條件。 |
| １、 | 年齡滿50歲以上【民國64年（含）以前出生】，即以「接種年」減「出生年」計算大於等於50歲。 |
| ２、 | 已於113年接種流感疫苗及新冠疫苗。 |
| ３、 | 已簽署預立醫療決定書（Advance Decision, AD）。 |
| (二) | 補助方式：提供1劑市自購非活性基因重組蛋白帶狀疱疹疫苗（下稱RZV）予實施對象免費接種。 |
| (三) | 接種原則： |
| １、 | 從未接種過RZV者，提供自費接種1劑RZV，同時安排間隔2至6個月內免費接種第2劑。 |
| ２、 | 曾接種1劑RZV且間隔至少2個月者，提供免費接種1劑RZV。 |
| (四) | 接種地點：本市13區衛生所。 |
| 三、 | 為能順利推動旨揭接種計畫，請貴校(園)協助運用網站或公布欄刊登訊息，電子宣導海報及計畫相關資料可逕至本府衛生局網站首頁＞業務資訊＞預防接種項下之帶狀疱疹疫苗補助計畫專區查詢（https://gov.tw/RCt）。 |
| 四、 | 如貴校(園)需實體海報（尺寸為A1)，請以電子郵件通知本府衛生局窗口：03-3340935分機2104，聯絡人：徐宗碩先生，E-mail：10024487@mail.tycg.gov.tw，並請提供寄送地址及需求數量。 |
