| 一、 | 依據桃園市政府教育局113年5月7日桃教特字第1130042243號函辦理。 |
| 二、 | 旨揭活動訊息如下: |
| (一)活動時間： 第1梯次：113年7月8日(星期一)至7月9日(星期二) 每日09：00-12：10 。 |
| 第2梯次：113年7月10日(星期三)至7月11日(星期四) 每日09：00-12：10。 |
| (二)活動地點：桃園市平鎮區復旦國民小學（桃園市平鎮 區廣平街1號）。 |
| (三)報名資格及錄取人數：本市112學年度一至四年級學 生，共計20~30名。 |
| １、順位一：以兩梯次全程參與為優先錄取，再依報名順 序錄取額滿為止。 |
| ２、順位二：無法參與兩梯次依然可報名各梯次，再依報 名順序錄取額滿為止。 |
| (四)報名時間：即日起至113年6月28日(星期五)止。 |
| (五)報名方式： |

| 2、現場報名:復旦國小輔導室。 |
| 3、紙本郵寄。 |
| (六)收件方式： |
| 「個人報名表暨家長同意書」(附件一)、紙本資料寄至 復旦國小輔導室（324桃園市平鎮區廣平街1號）或將 「個人報名表暨家長同意書」Word檔或PDF檔，寄至 電子郵件(fd195@fdes.tyc.edu.tw)輔導室輔導主任古重 頡收。 |
| (七)錄取名單：於113年7月3日（星期三）15:00前公告於本 校及舞蹈班網頁最新消息。 |
