| 說明： |
| 一、 | 依據教育部國民及學前教育署113年11月7日臺教國署秘字第1135901230號函辦理。 |
| 二、 | 旨揭評選本市收件日期自即日起至114年1月22日(星期三)止 ，送件前請務必詳閱推薦資料格式說明(如附件)，並郵寄以 下資料至本局終身學習科辦理，所附資料將由本局審查後函文承辦學校國立中興高級中學進行評選事宜： |
| (一) | 推薦資料核章正本1式4份，包含自主檢核及推薦單位複核表、推薦表、具體感人事蹟、佐證資料及會議紀錄(含簽到表)，並加製封面、目錄及封底裝訂成冊。 |
| (二) | 受推薦人「與學生互動照片」橫式3張jpg檔(單張像素不低於800萬像素)。 |
| (三) | 光碟1份(受推薦人員倘有2位以上，請分開燒錄)。 |
| 三、 | 本案請貴校(園)務必依最新修正之實施計畫、自主檢核表及推薦表、推薦資料格式說明及程序辦理，未符規定者，恕不受理。 |
