說明：
一、依據教育部112年2月16日臺教師(三)字第1122600448號函
辦理。
二、依「教育奉獻獎選拔及表揚要點」（下稱本要點）規定，本要點以公私立學校已退休或離職之非現職校長、教師及軍訓教官為選拔表揚對象。受推薦人應具有投入教育或學生輔導之志願服務相關工作之具體績效，並由各機關學校及經許可設立之民間團體組織或基金會，向所在地之直轄市、縣（市）政府提出推薦，最近3年內曾獲本要點或教育部其他相關規定獎勵者，不再受理推薦或給獎。
三、受推薦人資料填報注意事項如下：
(一)書面資料：
１、推薦表各欄位請詳實填寫、勿缺漏，並請務必留意字數限制（含空格、標點符號及英文字母），逾規定字數將無法完成資料上傳。
２、具體績優事蹟佐證資料，請以A4大小列印、燕尾夾固定，勿膠裝。
(二)光碟資料：
１、推薦表word檔。
２、具體績優事蹟佐證資料，請擇要掃描成pdf檔，每位受推薦人可上傳10個資料(單一資料大小限10MB以內，總資料大小限40MB以內)，檔名請簡述資料內容。
３、半年內「彩色半身照片」1張(電子檔)，檔案格式JPG，檔案大小800KB-4MB。
四、檢附「教育奉獻獎選拔及表揚要點」、推薦表電子檔各1份，亦可逕至本局網站( https://www.tyc.edu.tw/ 便民服務→檔案下載→公務檔案下載→輸入關鍵字「教育奉獻獎」)下載使用。
五、本案請踴躍推薦符合選拔表揚資格之所屬人員，並請於112年3月31日(星期五)前，將推薦表、具體績優事蹟佐證資料紙本及電子檔光碟各1份逕寄(送)至本局終身學習科承辦人彙辦，郵戳為憑，逾期歉難受理。
