| 主旨： | 有關本府衛生局辦理之「113年桃園市電子煙危害知識答題抽好禮問卷活動」，請貴校轉知並鼓勵學生及兒童青少年踴躍參與，請查照。 |
| 說明： |
| 一、 | 依據本府衛生局113年8月27日桃衛健字第1130082152號函辦理。 |
| 二、 | 為保護本市青少年免於菸品、類菸品危害及降低其使用率，本局製作「電子煙危害」衛教宣導圖卡3篇，希冀以圖象情境提升其電子煙危害相關識能，建立學生瞭解菸害防制法規定。 |
| 三、 | 旨揭活動資訊如下： |
| (一) | 活動網址：https://forms.gle/iJNwpr8VnQrUTsSa8 |
| (二) | 活動時間：即日起至113年9月30日。 |
| (三) | 抽獎訊息： |
| １、 | 每人僅限參加1次，如發現重複參加或冒用他人姓名參加活動，將取消得獎資格。 |
| ２、 | 獎項數量：統一超商禮券（200元共計25名）。 |
| ３、 | 預訂於113年10月17日（星期四）將符合抽獎資格之資料，以電腦系統隨機亂數產出中獎名單。中獎名單將於桃園市政府衛生局無菸網（https://nosmoking.tycg.gov.tw/）最新消息上公佈，不另行通知或取得參加者同意。 |
