| 說明： |
| 一、 | 依據財團法人許潮英社會福利慈善事業基金會（以下簡稱許潮英基金會）115年2月25日基115字第0206號函辦理。 |
| 二、 | 旨揭「圓夢飛翔」計畫摘擇如下： |
| (一) | 計畫目的：幫助具有專才的弱勢學生進行才藝學習，按月提供獎助學金。 |
| (二) | 補助時程及金額： |
| １、 | 115學年度為國小一年級至國小五年級學生：115年10月1日至116年9月30日；每月補助新臺幣（以下同）2,000 元整，共為期12個月，每人共計補助2萬4,000元整。 |
| ２、 | 115學年度為國小六年級學生：115年10月1日至116年5月30日；每月補助2,000元整，共為期8個月，每人共計 補助1萬6,000元整。 |
| (三) | 計畫聯絡人：王如芬總幹事，電子信箱hcilove@hci.org.tw 。 |
| 三、 | 本案申請期程自即日起至本（115）年4月30日止，申請書請郵寄至下列地址：105臺北市松山區南京東路五段60號11樓 ，收件人：王如芬總幹事。 |
| 四、 | 檢送許潮英基金會來文及旨揭計畫（含申請書）各1份。 |
